Parent or Legal Guardian's Phone Number Waiver of Liability and Assumption of Risk Agreement Royal Dream Experience Please Read Carefully Before Signing Waiver Requirement: All guests entering the premises of Royal Dream Experience, regardless of participation in activities, must sign a valid waiver. Guests under 18 must have the waiver signed by a parent, legal guardian, or an individual with power of attorney. A completed waiver is required to participate in any services or activities at Royal Dream Experience. No exceptions. Hold Harmless Clause: By entering Royal Dream Experience, located at 305 South Salisbury Terrace Suite B Lecanto Florida 34464, you agree to assume all risks associated with your visit. Whether or not a liability waiver has been signed, all individuals on the property knowingly and freely accept responsibility for any incidents, including but not limited to: injuries, accidents, slips and falls, allergic reactions, choking, or other unforeseeable occurrences. Royal Dream Experience, its affiliates, owners, employees, independent contractors, and vendors are not liable for: - Personal injury, illness, or accidents (including slip and fall) - Allergic reactions from food, beverages, costumes, cosmetics, or spa products - Transmission of illnesses or infections, including but not limited to lice, bacteria, or viruses - Missing or unattended children - Damage or loss of personal belongings Participation in Royal Dream Experience activities and services, including but not limited to spa treatments, themed parties, and events, is strictly at your own risk. Service Disclaimer: The services provided by Royal Dream Experience are for entertainment purposes only and are not performed by licensed cosmetologists. While we strive to use child-safe, non-toxic products, we cannot guarantee against potential allergic reactions. Parents/guardians are responsible for notifying staff of any allergies or sensitivities in advance. Our services, including hairstyling, manicures, pedicures, and makeup, do not involve the use of professional-grade tools, equipment, or chemicals. While every effort is made to ensure safety and quality, guests and their guardians must acknowledge the inherent risks associated with participation. Acknowledgment and Waiver of Liability: By signing this waiver, I: 1. Acknowledge that activities at Royal Dream Experience carry inherent risks, including potential injury, allergic reactions, and other unforeseen incidents. 2. Understand and agree to follow all posted rules and guidelines. 3. Release Royal Dream Experience, its owners, staff, and affiliates, from any claims of liability, including injuries or damages arising from negligence. 4. Assume full responsibility for myself, my guests, and any participating minors, including monitoring their safety and behavior. Photo Release Form: I, the undersigned, grant Royal Dream Experience permission to use photographs taken during events for marketing purposes, including but not limited to advertisements, social media, and web content, without expectation of compensation. COVID-19 Liability Release Form: I affirm that I, my child(ren), and all household members: - Have not experienced COVID-19 symptoms in the past 60 days. - Have not been diagnosed with COVID-19 in the past 60 days. - Have not knowingly been exposed to anyone diagnosed with COVID-19 in the past 60 days. - Have not traveled to high-risk areas within the past 60 days. I understand that while Royal Dream Experience takes precautions to sanitize and maintain a safe environment, there is no guarantee against exposure to COVID-19 or other illnesses. Acknowledgment of Understanding: I, the undersigned, certify that I am over 18 years of age and have the authority to sign this waiver. I have read and fully understand the terms of this agreement and assume all risks associated with participating in activities at Royal Dream Experience.
